Step 1: Agree to Licensing Terms

Before you can order, you need to agree to our licensing terms. We must have your signed agreement on file before we can accept your order. Please download and print the License Agreement. Read and sign it, then fax it to (319) 362-3906. You will need the free Acrobat Reader to read and print the agreement.

Please allow one business day after you fax your agreement before placing an order. If you include an email address in your signed License Agreement, we will send you an email when your agreement is approved.
